Fixed amount per year
Claims cannot exceed the amount per year limit
Can claim for the same condition again, excess paid once in each 12 month period
Is a suitable insurance for long term illnesses
Fixed amounts per year usually reset each year

Usually the most expensive insurance but gives the best cover for peace of mind.

Per Individual Condition

Fixed amount per condition
12 months cover and conditions not covered after limit is reached
Can only claim for condition once within the 12 month period (check with insurer as this term can vary)
Not suitable for long term illnesses

Cost somewhere between "Lifetime" and "Per Condition" with some limitations on claims in a 12 month period.

Annual

Fixed amount per condition
No claims can be made on the same condition once limit has been reached
Unable to claim for the same condition twice
Not suitable for Dogs with ongoing illnesses or multiple claims on the same illness

Usually the cheapest insurance but has limitations, especially with ongoing illnesses.
